As you prepare for your first appointment, please read the following insurance information carefully. Also, please download our Financial Policy (linked below and also on the Patient Resources page) and bring it with you to your first appointment.
Your insurance plan is a contract between you and your insurance company. As a courtesy, we will work with you to help you receive maximum benefits; however, your insurance company makes the final determination of your eligibility and financial responsibility. As a patient at Lake Cook Orthopedics, a Division of Illinois Bone & Joint Institute, you agree to pay any portion of the charges not covered by your insurance.
Some fees, such as co-payments, are due at the time of service. Lake Cook Orthopedics, a Division of Illinois Bone & Joint Institute will bill your insurance for the remaining cost. Please bring the necessary information needed to submit bills to your first appointment. This includes insurance cards, claim numbers, accident date (if applicable), case manager’s name and phone number (if applicable), and any other important information. If your insurance carrier is not listed above, contact our office or check your insurance’s provider directory, as insurance companies often change their networks. We attempt to maintain the list of participating insurance companies up to date; however, even if your insurance company is listed above, it is recommended that you confirm insurance coverage prior to your physician appointment.
