Our Occupational Therapists

Nicole Miller OTR/L, CLT

Nicole is a Wisconsin native, but has been calling Illinois home since 2007.   She is a 2004 graduate of the University of Wisconsin-La Crosse. Over her career she has worked primarily on treatment of the upper extremity in both the outpatient and inpatient setting. Her primary focus and area of interest continues to be rehabilitation of  upper extremity from the shoulder to the hand, including fractures, tendon injuries, post surgical rehab and orthosis fabrication. She is a member of ASHT, The American Society of Hand Therapists.  In addition to her focus in upper extremity rehabilitation, Nicole also became a certified lymphedema therapist in 2012 treating lymphedema and post-operative swelling.

Nicole is available to work with patients:

  • On Mondays and Wednesdays:  7:00am-3:30pm
  • On Fridays:  7:00am-1:00pm

Veena Vellookunnel, OTR/L

Veena graduated in December of 2013 with her Masters in Occupational Therapy from St. Catherine University in Minnesota. Veena always had a strong interest in orthopedic rehabilitation, sports medicine, and neurorehabilitation. She decided to complete a specialty clinical in hand therapy after graduation and previously worked on an inpatient rehab team where she gained experience with a variety of patients. Veena has a background in dancing, violin, and sports nutrition. She strives to provide client centered care and hopes to improve the patient’s experience by creating personalized treatment plans. She enjoys working with all age groups and demographics. She is a member of the American Occupational Therapy Association and Chicago Metro Hand Study Group. Currently, Veena’s main interests include rehabilitation of the upper extremity from the shoulder to hand and custom orthosis fabrication. Veena received her Bachelor’s degree in Exercise Science from Carroll University in Wisconsin.

Our Physical Therapists

Michael Sahli PT, DPT

Mike is a graduate of Carroll University’s Doctorate of Physical Therapy program and joined our team in May of 201. Mike is very interested in continuing to develop and progress his abilities, and consistently dedicates time to continuing education. Over the course of his academic career, Mike has developed a great interest in pain neuroscience, and its application to improving patient’s ability to return to their “normal” activities. He is interested in working with all age groups, and demographics and strives to provide best patient care. Mike has a background in sports performance, and worked as a certified personal trainer throughout his undergraduate studies. Mike received his B.S. in Exercise Science from Carroll University, where he was also a decorated member of the Carroll University Football Team. In an effort to give time back to the community, Mike volunteers his time as a Linebacker Coach to the Huntley High School, Varsity Football Team.

Dencel Polintan PT, DPT

Dencel Ray Polintan, PT, DPT has been practicing the profession since May of 2012. A graduate from the University of Santo Tomas in Manila, Philippines, he then received his doctorate degree from Utica College. He initially practiced in the Philippines at a musculoskeletal specialty clinic for 1.5 years. In Texas, he started as a home health physical therapist, catering to various cases. From there he transferred to Illinois and worked as an evaluating therapist in the skilled nursing setting. Dencel is interested in a variety of cases and is always ready to take on new challenges.

Dencel is available to work with patients:

  • On Tuesdays and Thursdays: 10:30am-7:00pm
  • On Fridays: 10:30am – 5:00pm

Melissa Hawkins PT, DPT, ATC, OCS

Melissa has worked as a Physical Therapist since 2006. With her previous work experience as a Certified Athletic Trainer, Melissa quickly took an interest to outpatient orthopedics and graduated with her Doctorate in Physical Therapy from Northwestern University. Melissa holds priority in Clinical Education and received credentialing as a Clinical Instructor by the American Physical Therapy Association in 2010. In addition Melissa opted to pursue advanced clinical skills by completing an Orthopedic Physical Therapy Residency program, successfully finishing the program in 2011, and subsequently passing the examination to become an Orthopedic Certified Specialist by the American Board of Physical Therapy Specialties in 2014. Melissa’s areas of interest include Sports Medicine and Orthopedic Trauma. She holds an additional certification as a Performance Enhancement Specialist. Melissa received her Bachelors degree in Exercise Science from The George Washington University in 2001, and when not treating patients, serves as a Reservist in the United States Marine Corps.
